About Hillcrest’s Legend Clinic:
Hillcrest’s Legend Clinic is the newest of the Hillcrest Family of clinics having opened our doors in December of 2009. We are conveniently located at the main hospital campus enabling easy travel to the hospital or other service departments to obtain other services such as Mammography, Radiology, etc, which are just down the hallway from our location. We also offer normal office visits, preventative care, minor procedures, and have laboratory and x-rays onsite.
Sunday/Holiday Clinic
The Legends clinic will operate every Sunday from 1 to 5 P.M. for established Hillcrest Patients. Patients are seen in the order they arrive at the clinic. Patients are seen on a walk in basis, therefore, there is no need to schedule an appointment. In addition to Sunday afternoons, the clinic will be open on most holidays (New Year's Day, Independence Day, Memorial Day, and Labor Day).
Important tems to Remember Before Your Next Appointment:
- Payment/copayment is required at the time of service.
- Please contact your pharmacy for refills, they will send a request to your physician.
- Be sure to make any refill requests to your pharmacy at least three (3) "business" days in advance of running out of your medication.
- Please contact us in advance if you are running late, or will not be able to make your scheduled appointment.

Please be prepared to provide this information to our clinic along with your current insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

Take IH-35 South, exit State Highway 6, staying on the frontage road, proceed under Hwy 6. Just past Hwy 6, turn right into the Hillcrest complex. The clinic is in Office Building I with a private entrance at the northeast corner of the building.
